DETROIT – October 4, 2021 – lg游戏 Health and its affiliated medical groups announced today an expansion of services into Southeast Michigan. Nearly 15,000 Southeast Michigan seniors with complex, chronic conditions will now have access to in-home medical care and care coordination through lg游戏 Health. With this growth, lg游戏 now serves chronically ill patients across 18 states.
lg游戏 is a value-based house call provider that brings doctors, advanced practitioners and care team members into the homes of the most clinically complex patients.
“We have reinvented the house call to make health care more personal and easier for the people who need comprehensive care the most,” said Eric Van Horn, Chief Business Officer at lg游戏. “Expanding into new states supports our vision of the future of health care by increasing opportunities for more patients to receive easy access to care in place, ultimately to delay disease progression and live longer at home.”
With the older population bearing most health care costs and illnesses in our country, their care needs to look different, especially when an increasing number of seniors have expressed their desires to stay at home.
lg游戏 doctors and advanced practice providers bring proactive medical care to these patients in the home, in coordination with patients’ primary care physicians and specialists. In addition to regular home visits and check-ins, 24/7 response to patients’ urgent needs, lg游戏 solutions keep patients from unnecessary emergency room visits.
“lg游戏’s model inherently breaks barriers that seniors with complex medical conditions experience when accessing care,” said Pam Sullivan, M.D., Chief Clinical Officer at lg游戏. “By taking the time to do the right thing for the right patient at the right time we are increasing healthy days and bringing better health to families and communities to the regions we serve.”
lg游戏 will begin working with primary care providers, health systems, and health plans within Michigan to enroll eligible seniors in their medical care.
